THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED OR DISCLOSED AND HOW YOU MAY ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This section applies when MDeed Health Solutions creates, receives, maintains, or transmits protected health information in circumstances governed by HIPAA.

The HIPAA Privacy Rule generally requires covered healthcare providers to explain how they use and disclose PHI, describe patient rights, state their legal responsibilities, provide privacy contact information, and provide an effective date. HHS.GOV

You may have the right to:

Obtain Your Health Records

You may request an electronic or paper copy of health information maintained about you as permitted by law.

We generally will provide access or explain why certain information cannot be provided.

Request Corrections

If you believe health information maintained about you is incorrect or incomplete, you may request that it be corrected or amended.

We may deny certain requests when permitted by law but will explain our decision.

Request Confidential Communications

You may ask us to contact you in a particular way or at a particular location.

For example, you may request that we contact you through a specific telephone number or email address when reasonably possible.

Request Restrictions

You may ask us not to use or disclose certain health information for treatment, payment, or healthcare operations.

We are not required to agree to every request, except where the law requires otherwise.

Request an Accounting of Certain Disclosures

You may request information about certain disclosures of your health information made during a period allowed by law.

You may request a paper or electronic copy of this Notice of Privacy Practices at any time.

Choose Someone to Act for You

If you have given another person medical power of attorney or another person is legally authorized to act on your behalf, that person may exercise applicable privacy rights for you.

You may contact MDeed if you believe your privacy rights have been violated.

You may also submit a complaint to the U.S. Department of Health and Human Services Office for Civil Rights.

MDeed will not retaliate against you for filing a privacy complaint.

Your Choices

For certain health information, you may tell us your preferences regarding how information is shared.

Depending upon the circumstances, this can include sharing information with:

• Family members

• Friends involved in your care

• Personal representatives

• Individuals assisting during emergencies or disaster situations

Certain uses or disclosures, including uses that require authorization under HIPAA, will not occur without appropriate permission.

MDeed does not sell protected health information in violation of applicable law.

How We May Use and Disclose Health Information

Where permitted by law, MDeed may use or disclose health information for purposes including:

We may use or share information with physicians, nurse practitioners, laboratories, pharmacies, healthcare facilities, or other professionals involved in your care.

We may use or disclose information when necessary for payment, billing, eligibility, or related administrative functions.

Healthcare Operations

Information may be used for activities necessary to operate healthcare services, including:

Information may be used or disclosed for qualifying research activities when HIPAA and other applicable requirements have been satisfied.

Substance Use Disorder Records

Certain records relating to substance-use-disorder treatment may receive additional protections under federal law, including 42 CFR Part 2.

Where those protections apply, MDeed will use or disclose such information only as permitted by applicable federal law, patient authorization, or another legally authorized basis.

Special protections may also apply to the use of certain substance-use-disorder records in civil, criminal, administrative, or legislative proceedings.

Our Responsibilities

When HIPAA applies:

• We are required to protect the privacy and security of protected health information.

• We are required to follow the privacy practices described in the current Notice of Privacy Practices.

• We will provide notice when required if a breach occurs that may compromise the privacy or security of protected information.

• We will not use or disclose PHI outside the circumstances permitted by law or described in our notice without appropriate authorization.

• When written authorization is required and provided, you generally may revoke that authorization in writing, subject to applicable legal limitations.

These principles are consistent with the current HHS model notice for covered healthcare providers.

Electronic Communications and Third-Party Platforms

MDeed may use secure electronic platforms to support healthcare services.

When directed to a secure patient platform, patients should use that system rather than general website forms, social media, standard email, or website chat to transmit confidential medical information.

Third-party vendors handling PHI on behalf of MDeed are expected to maintain appropriate privacy and security protections where required by law.

Changes to This Notice

MDeed may revise this Privacy Policy and Notice of Privacy Practices as its services, technology, legal obligations, or privacy practices change.

Revisions may apply to information already maintained by MDeed as permitted by law.

The current version will be made available on this website and will contain an updated effective date.
